COPD Self Management Pack

Helping patients to manage their chronic obstructive pulmonary disease

Self Management Plan coverBLF Services has produced a self-management pack for COPD. Offered for sale to healthcare providers, the pack is designed to:

  • Help patients to manage their chronic lung disease
  • Help reduce emergency hospital admissions - COPD is the second largest cause of emergency admission in the UK. It accounts for one million ‘bed days’ in hospital per year.

The pack is formed of two parts, the Self-management Plan and the Patient Records.

Part one, the Self-management Plan, explains what COPD is and how patients can manage their symptoms. It aims to give patients all the information they need to cope with COPD and improve their health.

Part two, the Patient Records, is used to record medical and other essential information. It is designed to be taken by the patient to medical appointments and to provide helpful information to their team of health care professionals. Part two also includes a peel-off oxygen alert card, used to make sure that when a patient has an exacerbation, or ‘lung attack’, doctors or ambulance teams do not give them too much or too little oxygen.

Patient recordsThe handy A5-size publications were produced in consultation with patients, respiratory nurses, GPs, consultants, physiotherapists and other healthcare professionals. The pack is an official product of the British Lung Foundation.

All publications can be produced to include organisational logos for an additional 20% on orders over 1000.

Prices start at £4.00 plus VAT per copy, with substantial discounts available for larger orders. Minimum orders are for 25 copies.
